The breeder, Dazzle Me Rattery (hereafter referred to as DAZL, 'our', 'we') is placing the

rat described above with the adopter described above as a companion.

While DAZL does not place animals that are ill, injured, or have serious temperament issues at the time of placement, no guarantee is expressed or implied about the health or temperament of this animal following placement to the adopter listed above.

The rat is not to be sold, given away, or otherwise disposed of or abandoned to any person, retailer, auction, institute or other entity. Should the adopter be unable or unwilling to care for the rat at any point, DAZL is to be contacted immediately and, if possible, arrangements will be made for the return of the rat, or assistance will be provided in finding a new qualified home for the rat

DAZL is to be notified in a timely manner in the event that the above-described rat develops a serious temperament problem, illness or chronic condition, or if any injury affecting quality of life is sustained, with the date of injury or when symptoms or problems began, as well as a detailed description of the problem

DAZL is to be notified at the time of the above-described rat's death with the date of death and a probable cause of death (if death was not naturally ocurring due to old age - a necropsy and/or pathology with a probable cause of death is preferred). This information is important, and it is not optional.

The above-described rat is to be quarantined to the best of the ability of the adopter for a minimum of three (3) weeks (twenty-one (21) days) to prevent the transmission of any parasites or potentially fatal diseases. True quarantine requires that the rat be housed in a separate building with a separate air supply from any/all other rodents.

The rat is to be cared for in a humane and responsible manner, provided with

A clean and adequately sized cage or enclosure appropriate for the rats age and/or size.

Companionship in the form of same-sex cagemates, or spayed/neutered rats

Nutritious and appropriate food

Clean water available at all times

Prompt veterinary care when necessary

Adequate human interaction and environmental stimulation

PET ONLY Contract
This animal is being adopted as a companion/pet ONLY and may not be bred under any circumstance. If this animal should become pregnant (if female) or sires a litter (if male), DAZL must be contacted immediately and all offspring placements must be approved by DAZL. DAZL reserves the right to reclaim this animal and any/all offspring resulting from any breeding at our discretion.
If necessary, DAZL reserves the right to temporarily reclaim this rat at any point in the future for breeding purposes if this should become necessary in improving our lines.

MALE BREEDING Contract
This animal may be bred by the adopter described above no earlier than (6) months of age, but preferably after (12) months of age. Potential mates must be approved by DAZL before any pairings take place. This animal may not be studded out to any other person without the express written consent of DAZL. If DAZL later decides that this rat or his/her line proves detrimental to the fancy, or DAZL decides the animal/line is no longer of good breeding quality, the adopter will be required to cease any and all breedings from this rat and/or line.
If necessary, DAZL reserves the right to the second pick of any and all litters resulting from breedings sired by this rat. DAZL also reserves the right to temporarily reclaim this rat at any point in the future for breeding purposes if this should become necessary in improving our lines.

FEMALE BREEDING Contract
This animal may be bred by the adopter described above no earlier than (6) months of age and (for a first litter) no later than (12) months of age. Should a second litter be necessary for improving the fancy, this rat must be given (3) months rest from the last litter's weaning date before attempting to breed her again. This female may not have more than (2) litters in her lifetime. Potential mates must be approved by DAZL before any pairings take place. This animal may not be studded out to any other person without the express written consent of DAZL. If DAZL later decides that this rat or his/her line proves detrimental to the fancy, or DAZL decides the animal/line is no longer of good breeding quality, the adopter will be required to cease any and all breedings from this rat and/or line.
If necessary, DAZL reserves the right to the second pick of any and all litters resulting from breedings of this rat. DAZL also reserves the right to temporarily reclaim this rat at any point in the future for breeding purposes if this should become necessary in improving our lines.

I hereby certify that all statements made by me on this adoption agreement are true and correct and that I agree to all terms of the contract. I agree that DAZL has the right to confiscate the above-described rat at their discretion in the event that any statements about me on the adoption questionairre I filled out are found false, there is reason to believe I may not be adequately caring for the rat, I have broken any portion of this contract, and/or my check for the adoption fee is returned for insufficient funds.

I understand and agree that DAZL makes no representations or warranties, expressed or implied, about the above mentioned animal’s temperament and/or health and is hereby absolved of any liability for future damages or expenses (veterinary or other) caused by this animal. Initial ______
